Introduction

Usually, I’ll be one of the first people you speak to when you call or visit the practice. As reception team members, we work to get patients the help they need as quickly as possible. We might ask about your issue so they can get you to the best person.

Social prescribing is about helping people with non-medical needs and giving practical and emotional support. As the name suggests, I help ‘link’ people to local community services and organisations. I meet with people to discuss the issues they’re facing, and what they would like to be doing, and together we set an achievable plan. We’ll then catch up regularly to see how things are progressing. This means people feel supported through the process, and I can offer advice and encouragement along the way.
