You do not usually need proof of address, immigration status, identification or an NHS number to register with a GP practice.

Information that may help
  • Your name, date of birth and current address
  • Your previous GP practice, if you know it
  • Your NHS number, if you know it
  • Information about medicines, allergies and ongoing care
